HOW TO ACQUIRE SHRAMAN AROGYAM PHYSICAL CARD?

  1. Download the Shraman Arogyam enrolment form from the website https://shraman.jito.org / Mobile App / Contact us
  2. Complete all fields in the form in BLOCK LETTERS
  3. Please ensure correct name and address of a responsible person where the card should be delivered
  4. Send completed form to JITO office by courier or in person (address available on the form)
  5. The form will be sent to Paramount office by JITO office for Registration
  6. Physical card will be ready within 15-20 days and will be sent to mailing address provided in the form
  7. Responsible person receiving the card must deliver the received package to the concerned Gurubhagwant ASAP
  8. This card is a permanent and perpetual card. It DOES NOT require to be renewed every year
  9. Along with the physical card, you will get a Directory, details on how to use the card and list of Network Hospitals where medical facilities can be accessed by Gurubhagwants
  10. Please ensure that the Gurubhagwant is made aware of how the card is to be used (details provided in the Directory)

HOW TO GET SHRAMAN AROGYAM E-CARD?

  1. In case of emergency ONLY, for Gurubhagwants who have NOT ENROLED in Shraman Arogyam, an E-Card can be made within one working day
  2. Online form submission is available at https://shraman.jito.org/phm-registration/
  3. Or you can EMAIL filled FORM to Shraman Arogyam Coordinator at [email protected] / [email protected] with Subject Medical Emergency-Urgently require E- CARD
  4. Details will be sent immediately to Paramount and E-Card will be generated within a working day
  5. In case of difficulty, please call JITO Apex office @ 022-42877736 Extn.: 714 / 8779531733 / 8655370257 and speak with Shraman Arogyam Coordinator for assistance
  6. E-Card can be used as soon as it is ready. Physical card will be ready within 15-20 days and will be sent to mailing address provided in the form
